To work in the field with students for Early Strides, I agree to the following:

I agree to be on time and to contact the case manager, clinical supervisor, scheduler, and the administrative supervisor in a group text if I will be late, absent or need to leave early unexpectedly. *

I will contact the administrative supervisor if an adjustment to my schedule is needed. *

I agree that if I need to change my availability, I will submit a request to the company’s administrative supervisor at least one month in advance. I understand that my request may not be granted. *

I understand that I will need to use my personal cell phone or computer to document time worked and Welligent, as designated by the company. I understand that the company will pay me a $5.00 stipend (per pay period) for using my personal device(s) on all pay periods worked. *

I understand that I must follow the company dress code policy while working, including during field trips. *

I understand that I must notify the case manager, clinical supervisor, scheduler, and the administrative supervisor as soon as I am aware that my student will be going on field trip. *

I agree to document session times in Welligent daily, before leaving the LAUSD school site. *

I understand that I need to complete a timesheet, mileage log, and student performance data on the company portal each day before leaving the LAUSD school site. I understand that I should also sign each entry, and each pay period. *

Attend all mandatory meetings and trainings at the company designated locations. *

Must follow all policies and procedures as outlined in the company employee handbook. *

I understand that I may not bring family members or pets to work with me. *

Check work email daily and respond within 24 hours. *

Respond to phone calls/emails/text messages from the company within 24 hours. *

I understand that subbing with another student is mandatory (within a 15 mile radius) when my student is absent. *

I agree to implement all treatment plans, behavior intervention plans, and crisis plans. *

I understand that the students I work with may have, but are not limited to excess behaviors such as: hitting, biting, slapping, pulling hair, head butting, kicking, scratching, grabbing, pushing, ripping clothes, punching, and/or internalized self-stimulatory and self-injurious behaviors. *

I agree to record all ABC data of problematic behaviors. *

I agree to correctly and consistently track all data on a physical data sheet and input into the Early Strides portal daily. *

I agree to vary reinforcements, as directed by the case supervisor. *

I agree to speak with my clinical team to address any environmental changes needed to best meet my student’s needs. *

Report any incidents (e.g. physical aggression towards peers/adults) involving assigned student to the case supervisor and teacher/school administration. Provide company documentation in accordance with the company policy and procedure handbook. *

Apply company procedures in accordance to Crisis Prevention Institute (CPI) to address crisis situations. *

I agree to meet my assigned student at the assigned area on campus. *

Direct all parent questions regarding student's behaviors to the case supervisor. *

Direct all parent questions regarding academics to the teacher. *

Refrain from providing personal contact information to parents, caregivers and school personnel. *

I understand this position requires the following physical job abilities: Body positions: sitting, standing and kneeling, Body movements: walking, running, bending and twisting, Body senses: the ability to see, hear and speak articulately, are required. *

I understand it is mandatory to sign in and out of the main office everyday. *

As a mandated reporter, call DCFS or police to report any suspicion of child abuse or neglect. Then complete required forms per company policy and procedure handbook. *

Distance Learning (if assigned to Distance Learning Program)

I understand that during any emergency school closure (i.e., the Covid-19 pandemic), I am required to work from home during my student's scheduled hours. I am to be at my home during working hours and I must be available during the entire scheduled time. *

I understand that I will be expected to return to the school campus once schools have been safely reopened. I will work at the school campus during scheduled times. I may also be working some hours from home on a hybrid schedule. *

I understand that I will be working with my student online during distance learning. *
